Job Title: Part Time Team Lead

CBI Health Group is seeking a dynamic and compassionate leader to join our team as a Part Time Team Lead. As a key member of our interdisciplinary team, you will play a vital role in coordinating smooth discharges from hospital to home, working with patients and families, and other community partners to create individualized and holistic plans of care.

Key Responsibilities:
  • Manage referrals and client intake
  • Work with hospital lead, patients and families, and other community partners to create individualized and holistic plans of care
  • Coordinate smooth discharges from hospital to home
  • Lead discharge and transition planning meetings
  • Participate in interdisciplinary discussions and huddles
Requirements:
  • Registered with the College of Nurses of Ontario in good standing (RN) or Registered with the College of Physiotherapists in good standing (PT) or Occupational Therapist registered with the College of Occupational Therapists in good standing (OT)
  • Minimum of 2-3 years of RN / PT / OT experience
  • Experience in home and community care or acute care
  • Excellent written and verbal communication skills
  • High degree of professionalism and patient/family focused care
  • Be a team player and be highly collaborative
  • Strong computer literacy with various applications and programs
  • Demonstrated clinical leadership & ability to lead discussions and collaboration
  • Excellent assessment and coordination skills
  • Demonstrated ability to work within an interdisciplinary team
  • Strong organization skills and able to prioritize/multi-task in a fast-paced environment
  • Access to a reliable and insured vehicle
  • A valid driver's license and ability/willingness to travel within York Region
  • Ability to communicate in French or other languages is considered an asset
