Job Description:

We are seeking a highly skilled Senior Clinical Pharmacist to join our team in a full-time temporary position until October 5, 2025. As a member of our interdisciplinary team, you will provide clinical and distributive pharmacy services to inpatients and outpatients within our computerized environment.

Your Key Responsibilities:

  • Evaluate medication orders and develop pharmaceutical care plans for patients
  • Resolve drug therapy problems and collect best possible medication histories
  • Facilitate medication reconciliation and ensure accurate dispensing of pharmaceuticals
  • Participate in risk management, patient safety, and quality improvement activities
  • Teach, mentor, and transfer knowledge to students, residents, and interns

Requirements:

To be successful in this role, you must possess a B.Sc. in Pharmacy and be registered with the Ontario College of Pharmacists (Part A). You should have at least two years of relevant pharmacy experience, preferably in a mental health setting or with a Hospital Pharmacy Residency and/or postgraduate pharmacy degree. Your skills and qualifications include:

  • Computerized unit dose drug distribution system experience
  • In-depth knowledge of pharmacy operations and medication-use processes
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work effectively with individuals from diverse backgrounds
