Job Description

The Access Navigator (Preschool Autism Resource Coordinator) plays a vital role in the Clinical Intake & Care Coordination stream of service, serving as the single point of contact for clients and families, as well as service providers at the point of referral.

Key Responsibilities
  • Initial Point of Contact: Families contacting PPAS are directed to the Access Navigator, who explores child and family needs upon entry into PPAS and reviews their goals/priorities as children and family needs change.
  • Navigating Child and Family Needs: The Access Navigator assists in matching child/family needs with the services available in PPAS, interacting with clinicians and administrative staff in PPAS foundational education, intervention, and diagnostic streams of service, as well as community-based organizations, partners, and clinicians involved in providing autism services to children and families.
  • Record Keeping and Communication: The Access Navigator maintains timely and accurate records of navigation and coordination activities for team communication and care coordination purposes.
  • Families and Relationships: The Access Navigator develops and maintains professional relationships with families throughout their preschool years, supporting the transition to school process and ensuring access to school-age diagnostic services as needed.
  • Interprofessional Collaboration: This position collaborates with an interprofessional team to plan, develop, and evaluate/quality improve services, promoting a respectful and consistently high-quality experience for clients/families, partners, and referring/community providers.
