Job Summary
The Ambulatory Program Specialist is a key member of the clinical team at Mackenzie Health, responsible for coordinating patient care and facilitating smooth transitions between healthcare services. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities
- Coordinate patient care and facilitate transitions between healthcare services
- Work collaboratively with clinical staff to ensure effective patient flow
- Provide clinical expertise and support to physicians, nurses, and other healthcare professionals
- Develop and implement strategies to improve patient outcomes and satisfaction
Requirements
- Registered Nurse with Bachelor of Science in Nursing (BScN) degree or equivalent
- Current general certificate of registration and member in good standing with the College of Nurses of Ontario
- Minimum of two years of relevant acute inpatient experience
- Excellent communication and interpersonal skills
