Home Support Team Lead

Spectrum Health Care, a renowned leader in home care services, is seeking an experienced Home Support Supervisor to join our dynamic team.

As a Home Support Supervisor with Spectrum Health Care, you will be responsible for leading a team of Personal Support Workers (PSWs) in providing high-quality patient care in the comfort of their own homes.
Responsibilities- Lead a team of PSWs to provide skilled personal care to patients in their homes, ensuring exceptional service delivery and customer satisfaction.
- Travel to patients' homes in community and facility settings, demonstrating adaptability and flexibility in your work schedule.
- Collaborate effectively with multidisciplinary teams in community and facility settings, fostering strong relationships and open communication.
- Assess patient needs and develop personalized plans of care, monitoring progress towards desired outcomes.
- Manage staff performance, conducting regular appraisals and addressing any issues promptly and fairly.
- Analyze feedback from patients and employees, maintaining awareness of key performance indicators such as caseload volume, staff turnover, occurrence rates, and satisfaction levels.
- Ensure timely resolution of patient concerns and complaints, documenting all interactions and actions taken.
- Promote Spectrum Health Care through participation in local healthcare or business committees and community events.
- Current registration with the Colleges of Nurses of Ontario in good standing (RN or RPN).
- A minimum of three years' experience in progressive leadership roles, preferably in a community setting.
- Exceptional interpersonal skills, enabling effective communication and conflict resolution.
- Strong writing and verbal communication skills, with the ability to articulate complex ideas clearly and concisely.
- Excellent English language proficiency (additional languages considered an asset).
- Solid knowledge of community nursing principles, practices, and methods, including program development, implementation, and evaluation.
- A valid driver's license and access to a reliable vehicle.
- Basic proficiency in Microsoft Office software, with the ability to learn proprietary systems.
- A competitive salary range of $65,000 - $85,000 per year, commensurate with experience.
