Job Summary

Southbridge Care Homes is seeking a qualified Chaplain to provide spiritual care and support to our residents, families, and staff. As a Chaplain, you will play a vital role in meeting the religious and spiritual needs of our community.

Key Responsibilities

  • Assess and provide direct spiritual support and care to residents, families, and staff members, ensuring access to spiritual, religious, and cultural practices/rituals as appropriate or requested.
  • Provide spiritual care and supportive counseling to residents, families, and staff, and/or make appropriate referrals to other members of the inter-professional team or community partners.
  • Lead prayer groups, study groups, church services, plan celebrations, and hold days for all faiths represented.
  • Participate in special bereavement functions, such as home-wide memorial services, at a minimum of quarterly.
  • Document care planning, goal setting, participation, and progress notes.
  • Coordinate, select, orientate, and evaluate other pastoral service providers for the facility.

Requirements

  • Successful completion of a degree or diploma in Divinity/Theology, Pastoral Education, or a combination of Spiritual education.
  • Demonstrated written and verbal communication skills, as well as excellent documentation skills.
  • Knowledge and demonstrated experience in one-to-one/group program planning, implementation, and evaluation for both well and cognitively impaired individuals.
  • Ability to work independently, take initiative, and manage time effectively.
  • Experience providing spiritual care in a long-term care setting is considered an asset.
  • Independent coverage for WSIB/insurance.
  • This position requires successful completion of a Police Check/Vulnerable Screening Check and Medical, dated within 6 months.
