Licensed Practical Nurse

The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for coordinating care delivery and ensuring the highest quality of life for our residents. As a LPN, you will be the role model and team leader for an assigned community, working closely with the Director of Care and other healthcare professionals to develop and implement individualized care plans.
Responsibilities- Assess residents' needs and develop and implement individualized care plans to meet their unique needs and preferences.
- Liaise with physicians, community agency representatives, families, visitors, volunteers, and other disciplines to ensure quality resident-centered care is delivered.
- Administer prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
- Monitor and report changes in physical and psychological conditions of residents, documenting changes on the resident health record and notifying family and physician as needed.
- Participate in Interdisciplinary Care Conferences and care plan reviews to ensure current and individualized care plans.
- Support the administrative requirements at the community, including admissions, discharge, and maintaining confidentiality.
- Actively participate in orientation of new staff, community programs, meetings, committees, and educational in-services.
- Provide coaching and direction to care staff as needed, using in-the-moment education to improve resident care.
- Maintain an up-to-date knowledge of community policies and procedures, particularly those concerning emergency procedures, equipment management, guidance, and assistance on safe work practices.
- Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
- Valid Occupational First Aid Level 1 certification.
- Successful annual Resident Assessment Instrument (RAI) competency testing.
- Previous experience in a long-term care facility is an asset.
- Ability to read, write, and speak English proficiently and understand verbal and written instructions.
- Computer skills, including the ability to navigate basic computer programs such as Word, Excel, and Outlook.
- Ability to push, pull, stoop, crouch, lift, and carry up to 25 lbs. Must be able to work with cleaners, detergents, and other cleaning equipment.
