Clinical Care Experience Supervisor

Bloom Care Solutions is seeking a highly skilled Clinical Care Experience Supervisor to join our team. As a Clinical Care Experience Supervisor, you will be responsible for leading a team of Personal Support Workers to deliver high-quality client-focused service.
Key Responsibilities:
- Assess and arrange for initial and ongoing assessments of clients requiring unlicensed care and service.
- Lead and direct a PSW team to deliver service experience that meets clients' needs and expectations.
- Develop and/or ensure that care plans are available for clients as appropriate.
- Conduct training sessions and facilitate and monitor training and educational opportunities for Personal Support Workers.
- Ensure that all clients' concerns and complaints are addressed and documented in a timely manner.
Requirements:
- A regulated health professional RPN with a minimum 2 years of experience.
- Preferably 2-3 years of recent experience with progressive responsibilities as a manager in the community setting.
- Solid knowledge of principles, practices, and methods of community nursing and service delivery to clients.
- Able to supervise a team of Personal Support Workers.
- Valid drivers' license and willingness to travel to Brantford, ON.
- Strong time management skills, confidentiality, and professionalism.
- Excellent communicator, able to foster relationships with Home Care Staff and clients.
- Ability to work independently and work effectively within a team environment.
- Interest in continuous learning – high energy and focused.
- Current CPR-C/BLS certification.
- Police Criminal Record Check with Vulnerable Sector Screening.
