Nurse Care Coordinator

The Nurse Care Coordinator shall possess the personality, ability, and temperament necessary to interact with individuals with diverse abilities in a respectful manner. This role is responsible for providing care and support within the scope of nursing practice and assisting in the daily operation and leadership of community inclusion services.
Key Responsibilities- Ensure the implementation, regular review, and updating of individualized program plans to address the total needs of each supported person.
- Communicate with fellow RNs/LPNs regarding assessments, required monitoring, necessary nursing care, and other relevant information.
- Update medication administration records (MAR) and medication change sheets as needed.
- Plan and evaluate nursing care and therapies for supported persons, collaborating with physicians, professional therapists, social workers, and other experts.
- Collaborate with team members without nursing backgrounds regarding medications, medication side effects, supported persons' diagnoses, and nursing care procedures.
- Continuously assess and manage care as needed.
- Advocate for the active participation of each person living in the community.
