Job Description

This position is covered under the HEABC wage levelling.

The Licensed Practical Nurse (LPN) reports to the Director of Care and/or designate. This role model and team leader coordinates care delivery for residents, ensuring individualized and resident-centered care within scope of practice, established standards, regulations, policies and procedures.

Responsibilities:
  • Assesses residents' needs; implements and evaluates care plans.
  • Ensures residents' rights are observed and adheres to a holistic philosophy of wellness.
  • Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to deliver quality care.
  • Assesses and treats wounds on the unit, acts as resource to care staff for strategies to prevent skin breakdown.
  • Administers prescribed medications and treatments within BCCNM limitations.
  • Monitors and reports changes in physical and psychological conditions of residents, documenting changes and notifying family and physician.
  • Active participant in Interdisciplinary Care Conferences.
  • Participates in care plan reviews to ensure current and individualized care.
  • Supports administrative requirements at the community, maintaining confidentiality.
  • Actively participates in orientation of new staff, community programs, meetings, committees and educational in-services.
Qualifications and Skills:
  • Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in long term care facility an asset.
  • All successful applicants must pass the vulnerable sector Criminal Record Check applicable to Provincial guidelines.
  • Ability to read, write, and speak English proficiently and understand verbal and written instructions.
Estimated Salary: $62,000 - $75,000 per year
