About the Role:

We are seeking a skilled Healthcare Transition Coordinator to join our team at William Osler Health System. In this role, you will play a critical part in ensuring seamless transitions for patients throughout their healthcare journey.

Job Description:

  • Patient Care Coordination: Work collaboratively with the inter-professional team to coordinate patient transfers and discharges, identifying high-risk discharges and developing effective discharge plans.
  • Risk Assessment and Management: Conduct thorough risk assessments using standardized tools and communicate results to the care team.
  • Care Planning: Meet with patients and families to determine pre-admission physical and mental functioning, identify community resources, and develop individualized care plans.
  • Leadership and Collaboration: Provide leadership and guidance to the care team, facilitate case conferences, and work with Home and Community Care Support Services to ensure timely discharges.
  • Documentation and Communication: Accurately document discharge plans, assessments, and progress, communicating effectively with patients, families, and the care team.

Requirements:

  • Education: Bachelor's Degree in a health-related discipline; post-diploma education/experience in gerontology an asset.
  • Experience: Minimum three to five years recent acute care hospital and community experience; one year experience in case management, utilization review or equivalent.
  • Regulated Health Care Professional: Member in good standing of a professional college; minimum one to two years in a leadership position.

Compensation and Benefits:

We offer a competitive salary range of $85,000 - $115,000 per year, depending on experience, as well as comprehensive benefits, including medical, dental, and vision coverage, pension plan, and generous vacation time.
