The Patient Care Observer is an essential role within the healthcare sector, focusing on the vigilant monitoring of individuals whose behaviors may pose risks to themselves or others. Committed to delivering personalized care, the observer engages in restorative practices and assists residents with their daily living activities.

Operating under the guidance of a registered healthcare professional, such as the Unit's Patient Care Manager or their designate, the patients under observation may include elderly individuals with dementia or younger patients recovering from traumatic incidents. This encompasses those affected by motor vehicle accidents, assaults, falls, or individuals who may have attempted self-harm or are dealing with physical disfigurements.

Key Responsibilities

  • Monitor assigned patients to foster a secure and compassionate environment.
  • Ensure the safety of residents, particularly during episodes of violent behavior.
  • Provide individualized supervision for residents with conditions such as Dementia, Alzheimer's, brain injuries, schizophrenia, and stroke.
  • Oversee residents at risk of elopement, including those on suicide watch, ensuring protection for both staff and residents exhibiting uncontrolled behaviors.
  • Engage residents in meaningful one-on-one activities, such as reading or storytelling.
  • Maintain close proximity to residents during supervision, ensuring constant support.
  • Complete necessary documentation as per facility requirements, including Dementia Observations Scale (DOS) and Behavior Assessment Record (BAR) every hour.
  • Alert registered healthcare professionals immediately if a patient is in danger, missing, or shows significant behavioral or condition changes.
  • Provide comfort and engage patients in cognitively stimulating recreational activities.
  • Uphold a professional appearance, including wearing a clearly visible name tag.
  • Assist patients with daily living activities under the direct supervision of registered healthcare professionals.
  • Communicate observations effectively with the healthcare team to aid in care planning.
  • Complete required documentation for Observers as outlined by corporate policies.
  • Perform additional duties as assigned, such as high-touch cleaning and other support tasks.

Qualifications and Skills

  • Minimum Grade 11 education or equivalent recognized by the Ministry of Education, Province of Ontario.
  • One year of relevant experience in patient care settings, or a PSW certificate from an accredited program, or completion of at least one year in an accredited Nursing program.
  • Fluency in English, both spoken and written.
  • Recent completion of an accredited Personal Support Worker certificate is an asset.
  • Demonstrated experience in delivering patient-centered care.
  • Ability to work effectively as part of an interdisciplinary care team.
  • Strong problem-solving, critical thinking, and communication skills.
  • Current Non-Violent Crisis Intervention Certification.
  • Proof of immunizations for measles, mumps, rubella, varicella, diphtheria, pertussis, and hepatitis B.
  • Proof of a negative 2-step TB skin test within the past year or a negative chest x-ray within the last five years.
  • Willingness to undergo a Vulnerable Sector Police Check as a condition of employment.
