Patient Care Observer

The Patient Care Observer plays a crucial role in ensuring the safety and well-being of individuals requiring close monitoring due to behavioral risks. This position is dedicated to providing compassionate, person-centered care while assisting residents with their daily activities.
Under the guidance of a registered healthcare professional, including the Patient Care Manager or designated supervisor, the observer will work with a diverse patient population. This may include elderly individuals with dementia or younger patients recovering from traumatic injuries, such as those resulting from accidents or assaults.
Key Responsibilities
- Monitor assigned patients to maintain a secure and supportive environment.
- Ensure the safety of residents, particularly during episodes of agitation or distress.
- Provide individualized supervision for residents experiencing cognitive challenges, including dementia and other mental health conditions.
- Oversee residents at risk of elopement or self-harm, ensuring their protection and well-being.
- Engage residents in meaningful activities to promote cognitive stimulation, such as reading or storytelling.
- Maintain close proximity to residents during supervision, ensuring constant oversight.
- Complete required documentation, including behavioral assessments and observation records.
- Alert healthcare professionals immediately in case of any safety concerns or significant changes in patient behavior.
- Interact with residents compassionately, providing comfort and engaging them in recreational activities.
- Uphold a professional appearance, including wearing a clearly visible name tag.
- Assist residents with daily living activities under the supervision of registered healthcare staff.
- Communicate observations effectively with the healthcare team to support care planning.
- Fulfill additional duties as assigned, including cleaning and equipment maintenance.
Qualifications and Skills
- Minimum educational requirement of Grade 11 or equivalent as recognized by the Ministry of Education, Province of Ontario.
- One year of relevant experience in patient care settings, or a Personal Support Worker certificate from an accredited program, or completion of at least one year in an accredited nursing program.
- Fluency in English, both spoken and written, is essential.
- Completion of a recent accredited Personal Support Worker certificate is an asset.
- Demonstrated ability to deliver patient-centered care effectively.
- Strong teamwork skills and the ability to collaborate within an interdisciplinary care team.
- Excellent problem-solving, critical thinking, and communication abilities.
- Current certification in Non-Violent Crisis Intervention.
- Proof of immunizations for measles, mumps, rubella, varicella, diphtheria, pertussis, and hepatitis B.
- Proof of a negative 2-step TB skin test within the past year or a negative chest x-ray within the last five years.
- Willingness to undergo a Vulnerable Sector Police Check as a condition of employment.
