Patient Care Observer

The Patient Care Observer plays a crucial role in providing attentive monitoring for individuals whose behaviors may pose risks to their own safety or that of others. With a commitment to personalized care, the observer engages in restorative practices and assists residents with daily living activities.

Under the guidance of a registered healthcare professional, including the Patient Care Manager or designated supervisor, the observer may work with a diverse range of patients, from elderly individuals with dementia to younger patients recovering from traumatic injuries. This includes those affected by incidents such as motor vehicle accidents, assaults, falls, or self-harm attempts.

Key Responsibilities

  • Monitor assigned patients to ensure a safe and supportive environment.
  • Guarantee the safety of residents, particularly during episodes of aggressive behavior.
  • Provide individualized supervision for residents with conditions such as Dementia, Alzheimer's, brain injuries, schizophrenia, and stroke.
  • Oversee residents at risk of elopement or self-harm, ensuring their protection and well-being.
  • Engage residents in meaningful one-on-one activities, such as reading or storytelling.
  • Maintain close proximity to residents during supervision, ensuring continuous support.
  • Complete necessary documentation as required by the facility, including behavioral assessments.
  • Alert registered healthcare professionals immediately in cases of danger, missing patients, or significant behavioral changes.
  • Interact compassionately with patients, providing comfort and engaging in stimulating recreational activities.
  • Present a professional image, including wearing a clearly visible name tag.
  • Assist patients with daily living activities under the supervision of registered healthcare professionals.
  • Communicate observations effectively with the healthcare team to aid in care planning.
  • Fulfill documentation requirements as per corporate policies.
  • Perform additional tasks as assigned, such as cleaning and preparation duties.

Qualifications and Skills

  • Minimum of Grade 11 education or equivalent as recognized by the Ministry of Education, Province of Ontario.
  • One year of relevant experience in patient care, long-term care, or complex continuing care settings is required, OR a Personal Support Worker certificate from an accredited program, OR completion of at least one year of an accredited nursing program.
  • Fluency in English, both spoken and written, is essential.
  • Completion of a recent accredited Personal Support Worker certificate is advantageous.
  • Proven experience in delivering patient-centered care.
  • Ability to work effectively as part of an interdisciplinary care team.
  • Strong problem-solving, critical thinking, and communication skills.
  • Current certification in Non-Violent Crisis Intervention.
  • Proof of immunization for MMRV, diphtheria, pertussis, and hepatitis B.
  • Negative 2-step TB skin test or chest x-ray within the past five years.
  • Willingness to undergo a Vulnerable Sector Police Check as a condition of employment.
