Job Overview

We are seeking a highly skilled and compassionate Community Home Coordinator to lead our team of Support Workers in providing exceptional care and support to residents in our Community Homes. As a key member of our team, you will be responsible for ensuring the effective delivery of services that meet the unique needs of our residents and their families.

Key Responsibilities:

  • Lead a team of Support Workers to provide high-quality resident support and care.
  • Develop and maintain individualized Resident Support Plans to ensure the best possible outcomes for our residents.
  • Provide regular communication and updates to residents' families to keep them informed and engaged.
  • Ensure the home is well-maintained and equipped with the necessary resources and supplies to support our residents' needs.
  • Provide ongoing training and development opportunities to Support Workers to enhance their skills and knowledge.
  • Offer regular coaching and feedback to Support Workers to support their growth and development.
  • Facilitate Person-Centered Planning Meetings to ensure our residents' needs are met and their goals are achieved.

Requirements:

  • Completion of Grade 12; related post-secondary education in Social Work, Psychology, Sociology, or a related field is considered an asset.
  • Strong organizational and time management skills, with attention to detail and ability to prioritize tasks effectively.
  • Comfortable with personal care and end-of-life care, with a commitment to providing compassionate and respectful support to our residents.
  • Experience working with individuals with intellectual disabilities, with a strong understanding of their unique needs and challenges.
  • Two or more years of supervisory experience, with a proven track record of leading high-performing teams and achieving excellent results.
  • CPIC with vulnerable sector clearance, as well as a valid Class 5 Driver's License.
  • TLR and PART certifications are considered assets, but not required.
