Lead Palliative Care Specialist

About the Role:
We're seeking a compassionate and experienced Nurse Practitioner to join our team as a Lead Palliative Care Specialist. In this role, you'll work collaboratively with inter-professional care teams across the healthcare continuum to provide high-quality, patient-centered care to individuals with complex palliative care needs.
Your Responsibilities:
- Provide direct care to patients with palliative care needs, ensuring rapid response capacity to address urgent issues
- Collaborate with primary care physicians and community nurses to provide expert clinical advice and support
- Conduct comprehensive clinical assessments, develop shared care plans, and coordinate interdisciplinary care
- Facilitate ethical decision-making and communicate care plans to patients, families, and healthcare teams
- Deliver education and professional development activities, interpret research findings, and recommend policy changes
- Lead quality improvement initiatives and program planning for palliative care services
Requirements:
- Current registration with the College of Nurses of Ontario in the Extended Class Nurse Practitioner Program
- Certification as a hospice palliative care nurse with the Canadian Nurses Association CPHCN(C)
- Minimum 2 years of experience in palliative care nursing, preferably in a community setting
- Valid Ontario driver's license and access to a vehicle
