Job Title: Community Rehabilitation Worker

We are seeking a highly motivated and compassionate Community Rehabilitation Worker to join our team at St. Paul Abilities Network (Society). As a key member of our team, you will play a vital role in supporting individuals with disabilities to achieve their goals and live independently.

Key Responsibilities:
  • Provide individualized support to clients with disabilities, focusing on their unique needs and goals.
  • Develop and implement personalized rehabilitation plans, working closely with clients, families, and healthcare professionals.
  • Assist clients with daily living activities, such as personal care, meal preparation, and household management.
  • Facilitate access to community resources, services, and activities that promote independence and social inclusion.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Document client progress, maintain accurate records, and contribute to quality improvement initiatives.
Requirements:
  • College diploma or equivalent in a related field (e.g., social work, rehabilitation, or disability studies).
  • Experience working with individuals with disabilities, preferably in a rehabilitation or support setting.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
  • Criminal record check and other required certifications.
