Key Responsibilities:
  • Provide individualized support to clients with disabilities, focusing on their unique needs and goals.
  • Develop and implement personalized rehabilitation plans, working closely with clients, families, and healthcare professionals.
  • Assist clients with daily living activities, such as personal care, meal preparation, and household management.
  • Support clients in accessing community resources, services, and activities that promote independence and well-being.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Document client progress, maintain accurate records, and participate in regular team meetings to discuss client outcomes.
Requirements:
  • College diploma or equivalent in a related field, such as social work, rehabilitation, or disability studies.
  • Experience working with individuals with disabilities, preferably in a rehabilitation or support worker role.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to client-centered care.
  • Valid driver's license and reliable transportation.
