Job Title: Home Care Nurse

CareHop Nursing & Home Care is seeking a skilled and compassionate Home Care Nurse to join our team. As a Home Care Nurse, you will provide high-quality nursing care to patients in their homes, promoting independence and improving their overall well-being.

Responsibilities:
  • Monitor patients' conditions and advise physicians of any changes
  • Assess patients to identify appropriate nursing interventions
  • Collaborate with healthcare teams to plan and implement patient care
  • Provide education and support to patients and their families
  • Administer medications and treatments as prescribed by physicians
Requirements:
  • Eligible for licensure as a Registered Nurse by provincial or territorial authorities
  • Valid driver's license and willingness to travel
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
