About the Role

VHA Home HealthCare is seeking a skilled Registered Practical Nurse to join our team in providing high-quality, client-centered care to individuals in their homes, schools, and community settings.

Key Responsibilities
  • Assess, monitor, and make care planning/coordinating decisions to ensure the best possible outcomes for clients.
  • Evaluate care plan interventions through ongoing reassessment and evaluation results.
  • Ensure the quality of care by identifying and acting on factors that affect practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.
  • Support client safety and care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Inform nursing supervisors of any client activities and relevant concerns or changes in the client or the client care plan.
Requirements
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Membership of your Professional Association preferred (WeRPN).
  • RPN diploma/degree from a recognized university or college.
  • Valid First Aid or CPR Certification.
  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
