Job Description

The Community Health Worker will serve as a vital link between patients, providers, and community resources. This role requires excellent communication and interpersonal skills, as well as the ability to navigate complex healthcare systems and connect patients with necessary resources.

Key Responsibilities

- Assist patients in understanding their health conditions and developing support strategies to improve their health outcomes.
- Connect patients with community resources and support services to address social determinants of health.
- Collaborate with care teams to facilitate plans that impact positive changes in care for chronic conditions and other health challenges.

Requirements

- Bilingual in English and Spanish.
- Strong working knowledge of focus populations served.
- Ability to maintain confidentiality and handle sensitive information with discretion.

Certifications and Trainings

- Completion or registration to complete CHW Professional Skills certification within first 2 years on the job.
- A valid driver license, verification of personal auto insurance, and the ability to become an authorized driver as detailed in the driver authorization procedure.
- CPR and Child/Dependent Adult Abuse Mandatory Reporter trainings are required within 6 months of hire and at renewal periods thereafter.
