About the Role

Vancouver Coastal Health is seeking a skilled Nurse Practitioner to join our team at the Three Bridges Community Health Centre. As a key member of our multidisciplinary team, you will provide comprehensive primary care to patients with complex medical and mental health needs.

Key Responsibilities

  • Assess and diagnose patients, developing treatment plans and prescribing medications as needed.
  • Collaborate with other healthcare professionals to provide holistic care and support.
  • Participate in quality improvement initiatives and contribute to the development of population-based strategies to improve health outcomes.
  • Stay up-to-date with the latest research and evidence-based practices, applying this knowledge to improve patient care.
  • Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.

Requirements

  • Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives.
  • Four years of recent, relevant clinical nursing experience.
  • Successful completion of the Objective Structured Clinical Examination (OSCE).
