We are seeking a highly motivated and compassionate Social Services Coordinator to join our team at Maison McCulloch Hospice. This is a full-time permanent position, offering a competitive salary range of $65,000-$80,000 per year, depending on experience.
Job SummaryThe successful candidate will be responsible for coordinating social services for patients and families, including volunteer coordination, client assessments, and case management. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
Responsibilities- Coordinate social services for patients and families, including volunteer coordination and client assessments
- Develop and implement individualized care plans in collaboration with multidisciplinary teams
- Provide emotional support and counseling to patients and families
- Recruit, train, and supervise volunteers
- Maintain accurate records and reports
- Bachelor's degree in a related field (e.g., social work, psychology, nursing)
- Minimum 5 years of experience in social services or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and crisis management skills
