The estimated salary range for this position is $42.56/Hr. - $52.95/Hr., commensurate with experience.

Job Description
We are seeking an experienced Case Manager to join our Home Health team working out of the South Health Unit in Vancouver, BC. As a Case Manager, you will work independently to provide case management services to clients with complex health care needs, including geriatric clients and adults with chronic illnesses. Your expertise will be instrumental in developing and implementing effective care plans, coordinating community resources, and ensuring continuity of care across the continuum.

Key Responsibilities
  • Conduct comprehensive assessments of physical, cognitive, psychological, social, functional, and environmental factors.
  • Develop and implement individualized care plans that address client needs and goals.
  • Liaise with multidisciplinary teams, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Identify and access community resources to support client independence and well-being.
  • Maintain accurate and timely records of client interactions and progress.

Required Skills and Qualifications
To succeed in this role, you will require:
  • Current registration with the College of Occupational Therapists of British Columbia (COTBC) or the College of Physical Therapists of BC (CPTBC).
  • Two years' recent experience working with geriatric clients and/or adults with chronic illnesses in rehabilitation or community settings.
  • Eligibility for membership in the Canadian Association of Occupational Therapists (CAOT) or the Physiotherapy Association of British Columbia (PABC).
  • Valid BC Driver's License for local area travel.
