Job Description

We are seeking a highly motivated and organized Occupational Therapist to join our team. As a skilled therapist, you will work with patients in their homes to develop and implement personalized treatment plans that promote independence and improve quality of life.

Responsibilities:

  • Conduct thorough assessments of patients to identify their strengths and needs.
  • Develop and implement individualized treatment plans that address patients' goals and objectives.
  • Provide hands-on therapy sessions to patients, including exercises, activities, and education.
  • Communicate effectively with patients, family members, and other healthcare professionals to ensure seamless care.
  • Document patient progress and maintain accurate records.

Requirements:

  • Master's degree in Occupational Therapy.
  • Current California licensure as an Occupational Therapist.
  • At least 1 year of experience in home health care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
