Canadian Mental Health Association - Job Opportunity
We are seeking a highly skilled and compassionate Case Manager to join our team in Aurora, Ontario. As a key member of our Mental Health & Justice Program, you will work closely with adults and youth (aged 12 years and older) who have complex mental health needs and involvement in the criminal justice system.
About the Role:
The successful candidate will be responsible for assessing individuals' strengths and needs, developing personalized long-term goals, short-term objectives, and concrete action plans to achieve these objectives. This will involve assisting clients in acquiring skills, connecting with formal and informal support systems, peer support, professional services, and resources to reduce the likelihood of further conflict with the law, enable progress toward recovery, and improve quality of life.
Key Responsibilities:
Assess and identify individual strengths and needs through collaborative work with clients.Develop and implement comprehensive treatment plans that address mental health needs and promote recovery.Collaborate with community agencies, healthcare providers, and court staff to ensure seamless service delivery and effective outcomes.Provide ongoing support and guidance to clients, addressing their concerns, and empowering them to make informed decisions about their care.Document client progress, maintain accurate records, and participate in regular case reviews to ensure high-quality service delivery.Requirements:
Degree or diploma in health or social services, or equivalent experience in a related field.Minimum four years of experience in a community-based mental health setting, preferably working with adults and youth with complex needs.Current registration with the OCSWSSW or a regulatory body under the Regulated Health Professions Act.Knowledge of systemic issues, social determinants of health, and mental health reform principles.Ability to apply recovery principles and empowerment-oriented philosophies and practices in work with consumers.A second language reflecting the local community is an asset.Satisfactory Vulnerable Sector Screening (Police Check).Educational Requirements:
Possession of a degree or diploma in health or social services, or equivalent experience in a related field, is essential for this role. Preference will be given to candidates with experience working in community-based mental health settings, particularly those with adults and youth with complex needs.
Work Environment:
This position requires working in an office environment, community settings, meetings in clients' homes, and travel with clients. A valid Canadian driver's license, minimum third-party liability insurance, and proof of insurance covering personal vehicle use for transporting clients are required.
Compensation Package:
We offer a competitive compensation package, including a salary range of $54,800 - $58,700 per year, based on experience and qualifications. The salary will be determined upon hiring.
