The Inpatient Low Tolerance Long Duration (LTLD) Rehabilitation Unit is a specialized care setting for patients requiring therapy as a result of musculoskeletal conditions. The primary objective is to facilitate patients' attainment of maximum physical and functional goals, while addressing emotional, social, and spiritual needs to enable them to return to living in the community as independently as possible.
Key Responsibilities:- Develop and implement individualized care plans, incorporating nursing interventions, including medication administration and treatments within the Registered Practical Nurse's scope of practice.
- Provide client-centered care within an interdisciplinary care team to meet the personal care, activation, recreation, and spiritual needs of clients, supporting their return to baseline functioning and discharge to the community.
- Collaborate with Registered Nurses as needed to develop and implement individualized care plans.
- Current registration in good standing from the College of Nurses of Ontario is required.
- Current Basic Cardiac Life Support certificate, Level C - Automated External Device is required.
- Previous geriatric rehabilitation experience is preferred.
- Excellent interpersonal, communication, and problem-solving skills.
- Basic computer skills, including Meditech, Microsoft Word, and Excel, are preferred.
