Key Responsibilities:
- Provide physiotherapy care to clients in a home-based setting
- Develop and implement individualized care plans to meet client needs
- Collaborate with multidisciplinary teams to ensure comprehensive care
- Conduct assessments and evaluations to identify client needs and goals
- Communicate effectively with clients, families, and healthcare teams
- Participate in ongoing education and training to stay current with best practices
Requirements:
- Current active registration with the College of Physiotherapists of Ontario in good standing
- Knowledge of legislation, regulations, and policies applicable to practice
- Knowledge of principles of reflective practice
- Ability to work within a flexible schedule and adaptable environment
- Valid Driver's License and reliable insured vehicle
- Vulnerable Sector Screening (VSS) within 12 months of hire (or in process), with a current Criminal Records Check
