Job Summary:

Closing the Gap Healthcare Group is seeking a skilled Physiotherapist to join our team of healthcare professionals. As a Physiotherapist, you will be responsible for providing rehabilitation support to clients in their homes, promoting independence and improving quality of life.

Key Responsibilities:

  • Provide physiotherapy services to clients in their homes, including assessments, goal setting, and treatment planning.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support.
  • Maintain accurate records and reports, including progress notes and treatment plans.
  • Participate in multidisciplinary teamwork to achieve client-centered care.

Requirements:

  • Current active registration with the College of Physiotherapists of Ontario in good standing.
  • Knowledge of legislation, regulations, and policies applicable to practice.
  • Knowledge of principles of reflective practice.
  • Ability to work within a flexible schedule and adaptable environment.
  • Valid Driver's License and reliable insured vehicle.
  • Vulnerable Sector Screening (VSS) within 12 months of hire (or in process), with a current Criminal Records Check.
