Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Durham Community Health Centre. As a key member of our primary care team, you will provide high-quality, client-centered care to individuals and families in our community.
Job Summary:The successful candidate will be responsible for delivering comprehensive primary care services, including health assessments, screenings, and treatment plans. You will work collaboratively with our interdisciplinary team to provide accessible, equitable, and culturally sensitive care to our clients.
Key Responsibilities:- Provide primary care assessments and develop treatment plans for clients with complex needs
- Conduct health screenings and provide health education to clients
- Collaborate with community agencies and healthcare providers to ensure seamless care coordination
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Provide clinical supervision and training to nursing students
- Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, Primary care
- Minimum of three years' experience working as a primary care NP with clients having complex needs
- Experience working with priority populations, such as Newcomers and ACB communities, is an asset
- Knowledge of relevant services and community supports in Durham Region is strongly preferred
