Job Title: Nurse Practitioner
Job Summary:
We are seeking a skilled Nurse Practitioner to join our team at Bayshore HealthCare. As a Nurse Practitioner, you will provide comprehensive primary healthcare to clients in high-priority neighborhoods through a mobile community-based model.
Key Responsibilities:
- Perform clinical assessments and therapeutic interventions to address the biopsychosocial needs of clients.
- Complete timely and accurate medical records of client encounters.
- Participate in chart reviews, quality improvement plans, and accreditation activities.
- Contribute to the development, implementation, and evaluation of medical directives, policies, and procedures.
- Provide primary care assessments, referrals, and connections to community-based and specialty services as indicated.
Requirements:
- Current and valid Nurse Practitioner license with the College of Nurses of Ontario (CNO).
- Completion of a Master's degree in Nursing.
- Prescribing Narcotics and Controlled Substances certificate from a CNO-approved training program.
- Minimum of three years' experience working as a primary care NP with clients having complex needs.
- Current and valid CPR certification.
- Experience working in a CHC setting or other community health practice with experience in community outreach and establishing community partnerships an asset.
- Flexibility to work in Oshawa, Whitby, Ajax, or Pickering as needed.
- Valid Ontario Driver's license and access to a reliable vehicle.
