Mental Health Case Coordinator

About the Role
Vancouver Coastal Health is seeking a Registered Nurse or Registered Psychiatric Nurse to join the Youth Integrated Care Team at Sunshine Coast Mental Health in Sechelt, BC, as a Mental Health & Addictions Case Coordinator Nurse.
Key Responsibilities
- Work as part of a multidisciplinary mental health and addictions care team to provide treatment and case management services that are both inclusionary and flexible for individuals with mental illness and concurrent disorders.
- Establish and maintain relationships with, and consult with, clients, their families and colleagues to provide appropriate treatment services based on best practices and client need.
- Perform key duties such as intake, mental health assessment, outreach visits as required, treatment planning, crisis intervention, consultation, individual counseling, education, and support groups to facilitate and coordinate client access to services.
About Sechelt, BC
Sechelt is a charming seaside community located on the traditional territories of the shíshálh (Sechelt) Nation. It is approximately 50 km northwest of Vancouver and is accessible by a 40-minute ferry trip followed by a 25-minute drive along Highway 101. The community offers a range of outdoor activities, including sightseeing, boating, diving, camping, and mountain biking, as well as several municipal parks, eateries, locally owned shops, boutiques, and galleries.
Qualifications
- Diploma in Nursing or Psychiatric Nursing.
- Two (2) years' recent related experience providing treatment services to individuals with mental health issues, or an equivalent combination of education, training and experience.
- It is preferred candidates have One (2) years recent related experience working with children and youth with mental health and/or addiction issues in a health care setting or an equivalent combination of education, training, and experience.
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Valid BC Drivers License. Local area travel may require the use of a personal vehicle.
