About the Role
Vancouver Coastal Health is seeking a skilled Mental Health Case Coordinator Nurse to join our team in Sechelt, BC. As a key member of our multidisciplinary mental health and addictions care team, you will provide treatment and case management services to individuals with mental illness and concurrent disorders.
Key Responsibilities
- Conduct mental health assessments and provide treatment planning and crisis intervention
- Establish and maintain relationships with clients, their families, and colleagues to provide appropriate treatment services
- Perform key duties such as intake, mental health assessment, outreach visits, treatment planning, crisis intervention, consultation, individual counseling, education, and support groups

Qualifications
- Diploma in Nursing or Psychiatric Nursing
- Two (2) years' recent related experience providing treatment services to individuals with mental health issues
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Valid BC Drivers License
