We are seeking an experienced Registered Practical Nurse (RPN) to join our team as an Intake Coordinator. In this role, you will play a vital part in assessing patient needs, determining eligibility for services, and developing individual care and service plans.
Key Responsibilities:- Collaborate with patients and their families to assess care needs and develop individual care and service plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
- Work effectively with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, and geriatrics
- Registered Practical Nurse in good standing with the College of Nurses of Ontario
- Relevant experience in long-term care homes or healthcare and/or in social services
- Experience working with seniors and/or physically disabled individuals
- Understanding of issues and priorities within the healthcare sector
- Knowledge of tools, systems, and databases used in healthcare service delivery and management
