About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in ensuring that our patients receive the highest quality care and support in the comfort of their own homes.

Key Responsibilities

  • Assess patient needs and develop individual care plans to meet their unique requirements
  • Link patients with service providers and coordinate care delivery
  • Monitor patient progress and make adjustments to care plans as needed
  • Establish strong relationships with patients, families, and healthcare providers to ensure seamless care coordination
  • Balance patient needs with available resources, prioritizing their values and preferences

Requirements

  • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
  • Relevant experience in long-term care homes or healthcare and/or social services
  • Experience working with seniors and/or physically disabled individuals
  • Understanding of healthcare sector issues and priorities
  • Knowledge of healthcare tools, systems, and databases
  • Practical knowledge of relevant legislation regarding healthcare services, privacy, and more
  • Effective planning, organizing, and evaluation skills to manage multiple patients and provide information reports
  • Ability to accurately complete required documentation, reports, forms, and recommendations
  • Strong interpersonal and communication skills to work with diverse patient groups
  • Effective listening and facilitation skills to work with patients, families, caregivers, or substitute decision-makers
  • Ability to deal with demanding interpersonal situations and respond with good judgment and understanding
  • Ability to communicate using various software programs to clearly articulate patient status
  • Strong collaboration skills to participate in projects and committees within the organization
  • Working knowledge of computer software (email, internet) and Microsoft Office applications
  • A valid driver's license and access to a reliable vehicle
  • Advanced oral and written proficiency in English is essential
