Care Coordinator Role

The Care Coordinator is a vital member of our team at Lifemark Health Group, responsible for delivering exceptional patient care and administrative support. This role requires a strong focus on customer service, organizational skills, and attention to detail.

Key Responsibilities

  • Provide warm and welcoming service to patients and visitors, ensuring a positive experience from arrival to departure.
  • Coordinate patient intake, data entry, and scheduling of appointments, maintaining accurate and up-to-date medical records.
  • Assist patients with completing paperwork, ensuring accuracy and minimizing errors.
  • Collect payments at the time of service and track coverage for all funding streams.
  • Process incoming and outgoing email, mail, and faxes, and coordinate courier packages.
  • Offer administrative support to the Care Coordinator Manager and Clinic Director.

Qualifications & Core Competencies

  • High School Diploma, G.E.D., or equivalent.
  • Excellent customer service skills and telephone etiquette.
  • High degree of organizational skills and ability to prioritize and multi-task.
  • Excellent communication (verbal/written) and interpersonal skills.
  • Strong computer and data entry skills.
  • Flexible and adaptable to change.
  • Experience in a Physiotherapy clinic an asset.
