About the Role:

Closing the Gap Healthcare Group is seeking a highly organized and detail-oriented Care Team Coordinator to join our team. As a key member of our team, you will be responsible for scheduling multiple workers with a large number of clients in a fast-paced environment.

Key Responsibilities:

  • Advanced organizational skills, scheduling, and multi-tasking are required for this position.
  • Accurately and efficiently assessing customer requests and addressing them immediately or escalating as appropriate.
  • Scheduling and coordination of service providers to meet client needs.
  • Strong problem-solving skills and effective communication with team members.

Requirements:

  • An outstanding commitment to providing excellent client-centered customer service.
  • Effective verbal and written communication skills.
  • Precise attention to detail and efficiency in data entry.
  • Knowledge of Medical terminology is an asset.
  • Computer and phone literacy.
  • Team-oriented and a diploma from Community College in Business Administration, Office Administration, or Medical.
