Occupational Therapist

Vancouver Coastal Health is seeking an Occupational Therapist to join our Primary Care Network team in Powell River, BC. As an Occupational Therapist with VCH, you will provide occupational therapy services to enable patients/clients to achieve and maintain optimal health outcomes.
Key Responsibilities- Provide occupational therapy services to patients/clients in a variety of community settings.
- Develop and maintain partnerships with Primary Care Providers in accordance with the local PCN care delivery model.
- Assess patients'/clients' occupational performance issues, needs, and goals, utilizing an evidence-based approach to plan and implement goal-directed services.
- Liaise with interdisciplinary teams to provide continuity of care and support seamless transitions in care.
- Contribute to the organization and delivery of occupational therapy services, and participate in departmental/program quality improvement initiatives, meetings, research, and education.
