We are seeking a highly skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.
Key Responsibilities- Assess patient care needs and develop individual care plans in collaboration with patients and their families.
- Link patients with service providers and coordinate care plan delivery.
- Establish a helping relationship with patients and their families, balancing their needs and choices with available resources.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
- Membership in good standing with the applicable regulatory body.
- 2+ years of recent experience in community health or a related field.
- Knowledge of the healthcare delivery system and community resources.
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
- Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment.
- Established ability to accurately complete required documentation, reports, and forms.
- A valid driver's license and access to a reliable vehicle.
- Proficiency in a Windows environment.
- Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
- Case management experience or recent related community experience.
- Ability to speak French or another second language.
