Job Title: Rehabilitation Assistant

About the Role:

We are seeking a highly skilled and compassionate Rehabilitation Assistant to join our team at Home and Community Care Support Services. As a key member of our healthcare team, you will play a vital role in providing therapy assistance to patients in their homes, promoting their independence and well-being.

Key Responsibilities:

  • Implement treatment plans with patients under the direction of registered therapists (PT/OT)
  • Assist registered therapists with patient assessments and treatment programs, adhering to College of Physiotherapists of Ontario (CPO) and College of Occupational Therapists of Ontario (COTO) guidelines
  • Utilize Sensory Tech Platform and other organizational platforms to receive clinical support from directing therapists
  • Complete and maintain documentation regarding patient intervention, adhering to Ontario Health atHome policies and procedures and CPO/COTO guidelines
  • Meet with registered therapists regularly to discuss patient treatment plans and progress
  • Research assessment, diagnosis, and intervention tools/aids/materials to assist registered therapists in delivering patient services
  • Promote maximum independence of patients in their homes, communities, and workplaces to support re-integration into the community
  • Carry out all duties in accordance with Ontario Health atHome policies and procedures and Guidelines for the Use of Supportive Personnel developed by CPO and COTO
  • Contribute to a safe and healthy environment by following safe work procedures, reporting injuries, illnesses, and unsafe working conditions
  • Promote patient safety in alignment with the Vision, Mission, Values, and Strategic Directions of Ontario Health atHome
  • Work within the basic principles of patient safety by doing the right thing for the right patient, using the right method at the right time
  • Adhere to Ontario Health atHome patient safety policies and procedures

Requirements:

  • Graduate of an accredited Physiotherapy Assistant/Occupational Therapist Assistant 2-year diploma program
  • PTA/OTA certification recognized by the College of Physiotherapists of Ontario (CPO) and the College of Occupational Therapists of Ontario (COTO) required
  • Previous experience as an Occupational Therapy Assistant/Physiotherapy Assistant in a healthcare setting within the last two years
