Job Summary
We are seeking a skilled Nurse Practitioner to join our team in Toronto (East Toronto). As a Nurse Practitioner, you will be responsible for providing high-quality nursing care to patients in the community. Your clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management will be essential in delivering patient-focused care.
Key Responsibilities
- Provide patient-focused, safe, and holistic nursing care and work collaboratively with other members of the healthcare team.
- Ensure that nursing care provided to our patients is based on best practices to achieve successful outcomes.
- Continually assess, anticipate, identify, and respond to new and changing needs of our patients.
- Travel within your region between patients' homes.
Requirements
- Minimum two years nursing experience (Community Nursing experience is ideal).
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- Ability to work independently in the community.
- Proficiency in English, both verbal and written required (additional languages an asset).
- Valid Ontario driver's license and reliable vehicle.
- Experience with infusion therapy, mental health, palliative care, and wound care.
- Excellent patient assessment and teaching skills.
