Prairie Mountain Health is seeking a skilled and compassionate Healthcare Navigator to join our team.

About the Role:

The Healthcare Navigator will play a vital role in providing exceptional care to clients by assessing their health and human service needs, planning and implementing personalized care plans, and coordinating services with other professionals and agencies.

Key Responsibilities:
  • Care Planning & Coordination: Conduct comprehensive client assessments, identify and prioritize needs, and develop tailored care plans that promote independence and well-being.
  • Clinical Analysis: Analyze data from assessments, identify areas of concern, and make recommendations for goal-oriented care.
  • Case Management: Manage client caseloads, monitor progress, and adjust care plans as needed.
  • Team Collaboration: Work closely with healthcare professionals, caregivers, and family members to provide comprehensive support and guidance.
Requirements:

To be successful as a Healthcare Navigator, you will need:

  • A degree in healthcare or a related field
  • Experience working in healthcare or social services
  • Strong assessment and problem-solving skills
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
What We Offer:

Prairie Mountain Health offers a competitive salary range of $65,000 - $85,000 per annum, depending on experience. In addition, we offer:
