At Prairie Mountain Health, we are committed to providing exceptional home care services to our clients. We are currently seeking a highly skilled and dedicated Home Care Case Coordinator Specialist to join our team.
About the Role
- As a Home Care Case Coordinator Specialist, you will be responsible for providing comprehensive case management services to clients in their homes, ensuring that they receive the necessary support and resources to maintain their health and independence.
- You will work closely with our multidisciplinary team, including healthcare professionals, social workers, and community resources, to develop and implement personalized care plans that meet the unique needs of each client.
- Your primary focus will be on intake and assessment, care planning, and monitoring and evaluation, ensuring that all clients receive high-quality care that is tailored to their individual needs.
Key Responsibilities
- Receive and review referral information, prioritizing and determining eligibility for Home Care services or re-directing the referral appropriately.
- Conduct comprehensive in-home client assessments to determine eligibility and care needs related to Home Care and/or Personal Care Home admission or alternative.
- Admit eligible cases to Home Care or re-direct appropriately, ensuring seamless transitions and minimizing disruptions to clients' care.
- Collaborate with clients, families, and healthcare professionals to develop and implement effective care plans that address the client's physical, emotional, and social needs.
- Monitor and evaluate client progress, making adjustments to care plans as needed to ensure optimal outcomes.
Requirements
- Bachelor's degree in Social Work, Nursing, or a related field.
- Minimum 2 years of experience in home care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient-centered care.
- Familiarity with community resources and healthcare systems, with the ability to navigate complex networks to secure necessary services and supports.
Compensation and Benefits
We offer a competitive salary of $60,000 - $80,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and vision coverage, retirement savings plan, and paid time off.
Location
This role is based in Winnipeg, Manitoba, with regular travel throughout the region to visit clients in their homes.
