Physiotherapist - St Thomas and Elgin County (Adults) - Home HealthCare Opportunity

VHA Home HealthCare is a leading provider of home care services in Ontario, and we are seeking a skilled Physiotherapist to join our team in St Thomas and Elgin County (Adults). As a Physiotherapist with VHA, you will have the opportunity to work with diverse client groups across Southern Ontario and the Capital region, providing physiotherapy consultation, assessment, and planned interventions.
Key Responsibilities:- Assess clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.
- Implement client interventions based on assessment information, client input, and in conjunction with the client's integrated care plan.
- Ensure that services provided are best practice, evidence-based, and evaluated through the routine use of valid and reliable outcome measures.
- Attend client-related meetings to report on assessment findings and discuss plans for intervention, progress reports, and discharge as required.
- Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
- Document all assessment findings, treatment plans, progress, and discharge summaries of interventions as required by the profession, referral source, and the organization.
- Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health, and other agencies.
- Baccalaureate or Masters degree in Physiotherapy from a recognized school of physiotherapy.
- Registered, in good standing, with the College of Physiotherapists of Ontario.
- Minimum of 2 years clinical experience, preferably in the community.
- Specialized courses to support areas of clinical expertise.
- Clinical expertise to provide a variety of physiotherapy interventions.
- Knowledge of community care requirements and resources available.
- Excellent consultation and teaching skills, and utilizes an evidence base to clinical practice.
- Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.
