Job Summary:
S.R.T. MedStaff is seeking experienced Registered Nurses (RNs) and Registered Practical Nurses (RPNs) to join our Community Visiting Nursing Program. As a Visiting Registered Nurse or Registered Practical Nurse, you will provide high-quality nursing care to patients in their homes, working collaboratively with other healthcare professionals to deliver comprehensive care.
Responsibilities:
- Assessment and Planning: Conduct thorough assessments of patients' needs and develop individualized care plans in collaboration with patients, families, and other healthcare professionals.
- Nursing Care: Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on self-care.
- Communication: Maintain effective communication with patients, families, and other healthcare professionals to ensure seamless care coordination.
- Advocacy: Act as a patient advocate, promoting patients' rights and dignity, and ensuring their needs are met.
- Education and Support: Provide education and support to patients and families on self-care, disease management, and other relevant topics.
Qualifications:
- Registration: Current registration with the College of Nurses of Ontario as a Registered Nurse or Registered Practical Nurse.
- Experience: One year of recent experience in community nursing, preferably in a visiting nursing role.
- Skills: Excellent problem-solving, time management, and crisis resolution skills, as well as strong interpersonal and communication skills.
- Education: Valid driver's license and reliable vehicle required.
