Social Worker

We are seeking a highly skilled Social Worker to join our Primary Care team at Northern Health Authority. As a Social Worker in this role, you will be responsible for providing comprehensive assessment, care planning, and intervention for individuals and families in our community.
Key Responsibilities- Conduct initial and ongoing assessments to identify client needs and develop effective care plans
- Provide direct psychosocial care, counseling, and group work to support clients and their families
- Collaborate with healthcare providers and the interprofessional team to ensure seamless care coordination
- Facilitate client self-management and provide education on relevant health topics
- Link clients to specialized or community resources as needed
- Bachelor's Degree in Social Work from an accredited university
- Two years of recent experience in healthcare or a related field
- Current registration with the British Columbia College of Social Workers
- Valid BC driver's license
