We are seeking a highly skilled Social Worker to join our Primary Care team at Northern Health Authority. As a Social Worker, you will play a vital role in providing comprehensive assessment, care planning, and intervention for individuals and families in our community.
Key Responsibilities- Provide direct psychosocial care, counseling, and group work to individuals and families
- Facilitate client self-management and provide education on relevant resources
- Collaborate with healthcare providers and the interprofessional team to provide comprehensive care
- Develop and implement care plans that address the unique needs of each individual
- Bachelor's Degree in Social Work from an accredited university
- Two years recent related experience in healthcare or a combination of education, training, and experience
- Current registration with the British Columbia College of Social Workers
- Valid BC driver's license
- Comprehensive benefit packages, including extended health/dental and a municipal pension plan
- Four weeks vacation with one year of continuous service
- Financial support for moving expenses for eligible positions
