Northern Health Authority is seeking a dedicated Social Worker to join our dynamic Primary Care team. As an integral part of our interprofessional healthcare network, you will play a vital role in supporting patients and families through comprehensive assessment, care planning, and intervention.
Your Responsibilities:
- Conduct thorough psychosocial assessments to understand patient needs and challenges.
- Develop individualized care plans in collaboration with healthcare providers and patients.
- Provide counseling, support, and advocacy to patients and their families navigating complex health issues.
- Connect patients with community resources and social services to enhance their well-being.
- Collaborate effectively with a multidisciplinary team to ensure holistic patient care.
Your Qualifications:
- Bachelor's Degree in Social Work.
- Minimum two years of experience providing social work services in a healthcare setting.
- Registration with the British Columbia College of Social Workers.
