Job Summary:

We are seeking a skilled Healthcare Transition Services Coordinator to join our team at Alberta Health Services. This is an exciting opportunity for a Registered Nurse to utilize their expertise in transition services, community resources, and acute care settings.

About the Role:

The successful candidate will be responsible for coordinating service delivery across programs/settings, liaising with Continuing Care staff and other community services/resources, and providing information and support to clients, families, and caregivers. The individual will also be required to develop and maintain relationships with Home Care Case Managers, Integrated and Supportive and Facility Living Case Managers, Facility leadership, and other agencies.

Responsibilities:

  • Completing comprehensive electronic needs assessments
  • Determining urgency/priority
  • Developing and implementing appropriate service plans
  • Coordinating service delivery across programs/settings
  • Liaising with Continuing Care staff and other community services/resources
  • Providing information and support to clients, families, and caregivers
  • Developing and maintaining relationships with Home Care Case Managers, Integrated and Supportive and Facility Living Case Managers, Facility leadership, and other agencies
  • Managing a caseload of clients while on the acute care unit who are current home care clients, new admissions to home care, clients needing complex discharge planning, and assessing clients for alternate levels of care

Requirements:

  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • Recent community experience with ISFL, Home Care or Transition Services
  • Recent Medical/Surgical Nursing experience
  • Excellent interpersonal, communication, conflict resolution, organizational, and critical thinking and assessment skills
  • Able to work with fixed deadlines, cope with frequent interruptions and changes
  • Able to perform physical requirements such as being on one's feet for an extended period (walking and standing)
  • Strong computer skills
