About the Role:

The Clinical Care Coordinator will play a critical role in ensuring the effective coordination and supervision of Community Health Services in Sault Ste Marie.

Key Responsibilities:

  • Provides leadership and guidance to Community Support Workers in the field.
  • Develops and maintains comprehensive Care Plans with respect to Community Health Services, working closely with Home and Community Care Support Services (HCCSS).
  • Stays up-to-date on policies outlined in the Regulated Health Professions Act and Community Health Services best practices, monitoring employee performance and providing coaching as needed.
  • Promotes and markets Community Health Services, maintaining positive public relations and collaborating with stakeholders.
  • Ensures active guidance for workplace Health and Safety, performing other duties as required.

Requirements:

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
  • Registration with the College of Nurses of Ontario (CNO).
  • Proven computer skills, including email, word processing, and work scheduling systems.
  • Thorough knowledge of case management skills and nursing processes and practices.
  • A valid driver's license and reliable vehicle for travel.
